Scheduling an Appointment When scheduling an appointment, please have your current insurance information and referral numbers available. |

Your Initial Appointment
When coming in for your 1st appointment, please bring the following:
Your current insurance information and referral numbers, along with your driver's license or comparable ID. We will need to photocopy these for our records.
Your insurance company's required copay, which we will collect as you check out after your appointment.
A list of your present medications and any known drug allergies.
Results of any tests done within the past year (CT scan or MRI films, etc.), that relate to your current condition.

It is very important that you hand carry your films with you to your appointment. Do not rely on either couriers or mail, for too often they don't get the films to us in time for your appointment. |

Follow Up Visits If this is a follow up visit, please make us aware of any changes in medications or insurance. |
 |
Cancellations If you must reschedule or cancel an appointment, please give us 24 hours notice so that we may give that time to another patient. |
