How do I reach my physician after-hours?
If you need to reach a physician after daytime office hours, please call your physician's office. Your call will be answered by our medical exchange. Your medical problem will be forwarded to the on-call physician. Please Click Here for the phone numbers of each of our locations. Please note: If you feel you are in an emergency situation, you should dial 911 or go immediately to the closest ER.

How do I get my prescriptions refilled?
When you need a prescription refilled, please contact the pharmacy, which will contact the office to obtain approval.
Please allow 3 days for local pharmacy refills and 5 days for mail order refills.

What information do I need to bring with me to my office visit?
You will need to bring your insurance cards, a list of all medications you currently take, and any medical records, test results, or films relating to your visit to us.
In addition, please come prepared with a Primary Care Physician Referral or Copay, as required by your insurance company.
If this is a follow up visit, please make us aware of any changes in medications or insurance.

How do I know if my insurance requires a referral or authorization?
If your insurance is an HMO, a referral by your Primary Care Physician is required. A good rule of thumb about this is if you were required to choose a Primary Care Physician when you signed up for benefits, you probably need a referral.

How do I know if my insurance will cover the test I'm receiving?
Most tests are covered by your insurance company as long as your physician believes it is medically necessary. If you have questions about coverage specifics, please call your insurance company.

How do I get copies of my medical records?
To release your medical records to you, we need a signed consent form which includes you name, your doctor's name, and the address to send the records to.
To allow adequate time for processing and mailing, please give us 2 weeks notice. Medical records are copied only on Fridays. They are mailed out the following Monday. Records may take at least a week to reach their destination.
